  • Short term illnesses that require temporary, daily medication during school hours (ex. Treatment for strep throat or ear infection, steroid taper, etc)

If your child requires daily prescription medication, please fill out the form below with all relevant information, Please be prepared to provide the nurse with a physician’s order and the medication in its ORIGINAL packaging, with the child’s name, dosage, and instructions clearly visible. The district will not be responsible for administering the first dose any medication for safety purposes.

OTC medications will also be available for illnesses or injuries that occur at school. If you would like your child to be able to take OTC medication should it be needed, please fill out the appropriate form (found below, or in the registration packet). If your child requires OTC medication more often (such as for chronic headaches that may be triggered during school hours), please be prepared to provide his/her own bottle with a physician’s order and the medication in its ORIGINAL packaging and fill out form below.

**Please be aware that it is not healthy or safe for children to take medication frequently or unnecessarily, and OTC medicine will not be used frequently. Furthermore, the district is not responsible for first aid that should be given at home. If your child has an ailment before or after school, such as headaches, stomach ache, a sports injury, etc, do not send your child to school and tell them to take medication in the nurse’s office. It will not be given without verbal request from parents. If there is an extenuating circumstance, please contact the nurse in your child’s building and arrangements can be made. If you cannot afford OTC medication to keep at home, please notify the nurse and arrangements can be made.
